I/We request that the child named below is registered as a Prospective pupil at St Dunstan's College ('the College').

I/We have made payment of the £100 non-refundable Registration Fee by WisePay. I/We acknowledge that registration does not constitute an offer of a place at the College.

I/We have disclosed any special circumstances relating to my/our child including details of any known medical conditions, Special Educational Needs and Disabilities ('SEND') as well as any behavioural, emotional or social concerns (both diagnosed and non-diagnosed) which may affect my/our child's ability to fully participate in the admissions process and educational provision of the College.

I/We understand that the College requires this information so that they can consider what reasonable adjustments, if any, the College can make in order to accommodate my/our child in the admissions process and should an offer of a place later be made.

I/We understand that the College's admissions criteria applies equally to all pupils irrespective of any SEND and acknowledge that an offer of a place may not be made if the College determines that they are unable to provide for my/our child's needs.

I/We understand that the College's facilities (physical and otherwise) for pupils with SEND are currently limited.

I/We understand that if I/we have concerns over whether the College is able to provide for and meet my/our child's needs that these must be fully disclosed and discussed with a member of senior staff before I/we accept an offer of a place should an offer later be made.

I/We understand that it is College policy not to offer any feedback on results or performance in the entrance assessments.

I/We understand that the College welcomes pupils of all faiths but it is expected that all pupils will fully participate in College events irrespective of individual beliefs (subject always to the College's obligations under the Equality Act 2010).

I/We consent on behalf of my/our child (so far as I am/we are entitled to do so) to the College processing my/our child's personal data (including sensitive personal data) for the purposes of administrating its list of prospective pupils and for the purposes of assessment, including seeking references from my/our child's previous educational establishment (if any) and confirmation that all fees have been paid.

I/We understand that the College may obtain, process and hold personal information about my/our child which may include sensitive personal data such as medical details, or reports relating to SEND and I/we consent on behalf of my/our child (so far as I am/we are entitled to do so) to the College processing such data for the purposes of assessment and, if an offer of a place is later made, in order to assess what reasonable adjustments, if any, are needed to meet my/our child's needs.
